Caregiver support from a geriatric care specialist qualified to provide consultations, referrals, and facilitation of family discussions regarding care

An estimated 44 million Americans provide care to an aging family member. These care requirements have a direct impact on employee productivity and employer costs due to lowered productivity and time away to complete caregiving chores.

 

Employees are often torn between their duties at work and their responsibilities as a caregiver of an older adult. Long distances and the lack of family agreement about care plans can make these responsibilities even more challenging. Advantage Care Coach can provide practical solutions to local and long-distance caregiving challenges by joining families with a Geriatric Care Manager (GCM).

 

An alternative to costly face-to-face geriatric care management services, Care Coach allows for families to hold a telephonic "family meeting" with a qualified GCM. The Care Manager conducts a detailed interview of family members and establishes a care plan that all parties can agree upon. Even in instances where a facilitated family meeting is not desired or appropriate, the GCM can help caregivers create a care plan for their care recipient that also "cares for the caregiver."

 

 

What Is a Geriatric Care Manager (GCM)?
A Geriatric Care Manager is a professional, such as a gerontologist, nurse, social worker, or psychologist with a specialized focus on geriatric care. GCMs work with older adults and their families to create a care plan that meets the needs of the older adult. Our GCMs meet the qualifications for membership of the National Association of Geriatric Care Managers and are members in good standing.

 

 

Consultation and Referral
Integration with our consultation and referral program makes Care Coach even more valuable to the caregivers employed by your organization. Our consultants are experts in finding localized care options and can offer a personalized service that focuses on providing caregivers with all the information they need to make informed and effective choices, whether it is locating vacancies in a skilled nursing facility or finding local Meals-on-Wheels programs.

How the Service Works

 

  • Employees access a consultant via the toll-free number.

  • An appointment is made for a family care conference facilitated by a GCM.

  • All interested family members are invited to join the care conference by way of a toll- free conference call-in number.

  • The GCM assesses challenges for both the care recipient and caregiver, evaluates where additional support is needed, and identifies available resources.

  • Discussion includes social supports, medical needs, physical functioning, personal care abilities, living environ ment, psychological and cognitive functioning, and any legal or financial issues.

  • The GCM creates a care plan, which includes recom mendations based on the findings.

  • A follow-up call is placed to family members to discuss the care plan and answer questions.
