Geriatric Care Assist

In-person or telephonic elder care consultation, assessment, and referrals provided by a geriatric care specialist

Advantage Geriatric Care Assist provides employees caring for aging family members the customized support services and practical solutions for their loved one's safety and their own peace-of-mind.

 

Customized Solutions
Offered as an employer-sponsored benefit at no cost to the family, our program provides a pre-determined amount of time with a professional geriatric care specialist. The geriatric specialist can meet with families, complete an on-site assessment of the elder's own living environment, or assess possible housing options. Working together, the family and the specialist decide the best way to use the resources of the Geriatric Care Assist benefit.

 

The geriatric care specialist will then prepare a detailed written report with care recommendations and an implementation plan. This plan includes a range of services tailored to create a safe, healthy, enriching lifestyle for the elder loved one. These services may include coordinating and monitoring home health services, household maintenance, lawn care, meal preparation, grocery delivery, and transportation solutions.

 

While our geriatric specialists do not mow the lawn or prepare the meals, they do provide referrals to the organizations and businesses providing those services. If the time comes when living at home is no longer an option for the elder, we will work to find the best alternative, including assisted living or nursing home care.

 

Trained Specialists
The credentials and backgrounds of each geriatric care specialist are carefully screened. What's more, this program has experts located across the nation. So, even if employees can't live near a parent or loved one, we can provide a caring professional who does.

How the Service Works

 

  • Employees access a consultant via the toll-free number.

  • A geriatric care specialist contacts the employee to set up an in-person or telephonic consultation.

  • The geriatric care specialist meets with the employee or family and performs an on-site assessment of the elder's living environment and possible housing options.

  • The family works with the geriatric specialist to determine what resources would benefit the elder, and how best to use the specialist's time.

  • The specialist identifies community resources qualified to provide the services required by the elder, and is available to monitor care and locate alternatives if necessary.
