Supporting the Return to Work after Cancer: A Holistic, Personalized Approach

Jorge de Sousa

Service Center Manager of Lisbon

The return to work after cancer is a growing priority in today’s workplace, and its significance has long been recognized. Globally, hospital-based programs provide psychological, occupational, and physical rehabilitation support to help patients return to life and work. While helpful, these interventions often lack a personalized and structured focus. At the employer level, resources and support for employees affected by cancer remain limited.

Returning to work after cancer often comes with significant psychological challenges. Many employees struggle with feelings of guilt or internalized stigma—seeing themselves as “cancer patients” rather than as capable workers—which can hinder their ability to contribute meaningfully at work. At the same time, the pressure to provide for themselves or their families creates another layer of stress. Unfortunately, many do not feel adequately supported by HR or their managers, and frequently express frustration over the limited information and resources available to help them navigate employment after a diagnosis.

Return to Work: Cancer Care Compass – A Personalized, Holistic Approach

With these challenges in mind, Workplace Options (WPO) has developed a specialized program to support both employees and their managers in ensuring a smooth and successful return to work. Returning to work is a deeply personal process of rebuilding confidence and capability, and demands a personalized, holistic approach. Grounded in the LLSW model (Learning, Living, Social, Working), Return to Work: Cancer Care Compass supports whole-person recovery by addressing not only physical and emotional health, but also social and professional wellbeing. It offers personalized care through every stage of the journey, from diagnosis and treatment to workplace reintegration, led by a dedicated Cancer Care Coordinator.

By integrating principles of vocational rehabilitation, Cancer Care Compass also helps individuals reclaim their professional identity and return to work with renewed purpose, confidence, and control. It empowers organizations to foster a culture of empathy, inclusion, and resilience. With structured, personalized guidance, the program bridges a critical gap in cancer recovery support: offering a comprehensive path forward for both employees and their managers.

Why Manager Support Matters

A return-to-work adaptation plan that addresses these considerations is crucial for a successful transition. During what is often a disorienting and vulnerable time for survivors, managers and HR professionals must be prepared to offer informed, compassionate support. Direct managers are responsible for providing day-to-day guidance and encouragement, while HR plays a vital role fostering a safe and supportive environment and ensuring that policies and resources are clearly communicated and accessible.

Cancer and Work: An Ethical Concern

Cancer survivors face a range of challenges upon returning to work. These include issues with fatigue, cognitive impairment, anxiety or depression, which can last well beyond the end of treatment. Workplace discrimination is also a troubling reality for many.

Research has demonstrated that several cancer-related symptoms are associated with poorer work outcomes, with fatigue and depression notably linked to diminished job performance. Despite these hurdles, it is imperative that employers provide robust support to employees who wish to return to work.  A study conducted by Debbie Kane, Dale Rajacich, and Chantal Andary from the Canadian Association of Nurses in Oncology found that most cancer survivors surveyed felt returning to work restored their sense of normality, personal identity, and purpose. Similarly, research funded by the German Cancer Aid showed that employed cancer survivors experience significantly lower levels of depression, anxiety, and distress compared to those who are unemployed.

Given these findings, returning to work after cancer should be regarded as a long-term process rather than a one-time event. Healing takes time and is rarely linear. Employers must commit to embedding meaningful, sustained support for cancer-affected employees into the fabric of their culture. This commitment not only aligns with principles of social justice and ESG priorities but also helps attract and retain top talent who value well-being and seek out ethically driven employers. Beyond that, a supportive workplace culture can help foster trust, boost productivity, and contribute to a healthier, more balanced workplace.

Cancer and Work: A Legal Concern

While supporting employees with cancer is a moral and ethical imperative, it is also a legal requirement in many countries. In the United States, for example, federal protections such as the Rehabilitation Act and the Americans with Disabilities Act (ADA) safeguard the rights of employees diagnosed with cancer. The Return to Work: Cancer Care Compass program offers comprehensive, country-specific guidance on legal rights—delivered by real people to ensure both employees and employers understand their responsibilities, entitlements, and available accommodations. This information is also accessible anytime via the program’s dedicated support hub.

Reintegration Reimagined

The Return to Work: Cancer Care Compass program honors the reality that every cancer journey is different—and so is the path back to work. Through personalized care plans for employees and tailored resources for managers, this program moves beyond limited, one-size-fits-all approaches to provide thoughtful, individualized support that meets the unique needs of each survivor and their workplace.

By facilitating a compassionate and compatible return-to-work experience, Return to Work: Cancer Care Compass empowers cancer survivors to rebuild their lives with confidence and clarity, while enabling employers to foster more inclusive, resilient workplaces.
